Job Description:
A company is looking for a Senior Care Manager (RN) to assess,
plan, and implement complex care management activities for
members.
Key Responsibilities
Develop and continuously assess personalized care/service plans for
members
Coordinate care between members, families, and healthcare providers
to ensure person-centered services
Monitor and revise care plans based on member status and progress
towards goals
Required Qualifications, Training, and Education
Degree from an accredited school of nursing or a Bachelor's degree
in nursing
4-6 years of related experience in care management
Registered Nurse (RN) state licensure or compact state licensure
required
For Illinois Youth Care Contract: Bachelor's degree in nursing
required
Preferred candidates to live in/around San Antonio or Austin,
TX
